Foley Catheter Change Procedure

Informed patient that foley cath will be changed. Using clean gloves, deflated foley cath balloon and removed __ ml of water. Instructed patient to take a deep breath and then slowly removed the old catheter in one motion without resistance or discomfort. Opened the insertion kit. Using aseptic technique, removed and placed the underpad beneath the patient, plastic side down. Positioned the sterile fenestrated drape around the patient’s genitalia. Donned sterile gloves and used strict sterile technique for the foley insertion procedure. Cleansed tip of penis/urethral area with iodine x 3. Using the dominant, sterile hand, covered the tip of the catheter with lubricant, then inserted the foley through the urethra into the bladder. Instructed the patient to inform the SN if any discomfort is felt with inflation of the balloon. If discomfort is felt, the catheter is most probably in the urethra and will need to be deflated and advanced. Inflated the balloon slowly, using the entire __ ml of sterile water. No complaint of discomfort from patient. Withdrew the catheter slowly to the point of resistance at the bladder neck. Patient tolerated procedure well without any complaint of discomfort or pain. Urine started draining from foley tip. Attached foley cath to 2000ml drainage bag. Instructed patient/cg to call SN if no drainage is observed after 3-4 hours.
