Port-A-Cath - Access Change

Current port-a-cath dressing observed to be clean, dry and intact. Removed old port dressing and de-accessed catheter without difficulty. Port site observed to have no redness, drainage, or swelling. Using sterile gloves, cleansed site with chloroprep, air-dried, accessed with a pre-primed _____" Huber needle x __ attempt without difficulty, established blood return immediately before flushing with 10 ml NS, secured with window-dressing, and used 5 ml of Heparin as last flush. Date of dressing change indicated on provided tape. Patient tolerated procedure without significant discomfort.
