Port-A-Cath - Monthly Flush

Informed patient/caregiver that skilled nurse will be accessing port today for monthly port flush. Port site observed to have no redness, drainage, or swelling. Using sterile gloves, cleansed site with chloroprep, air-dried, accessed with a pre-primed 1" Huber needle x 1 attempt without difficulty, established blood return immediately before flushing with 10 ml NS, secured with window-dressing, and used 5 ml of Heparin as last flush. Informed patient that flush is completed and that port will be de-accessed at this time. Removed huber needle without difficulty. Covered site with 2x2 gauze and secured with tape. Patient tolerated procedure without significant discomfort.
