Wound Vac - Dressing Change

Old dressing is clean, dry and intact. Turned off vac machine at this time. Clamped both tubings. Disconnected canister tubing from trac pad tubing. Canister observed to be full at this time. Removed old canister and discarded. Replaced with new canister and set aside for now. Old dressing removed slowly by peeling off drapes on surrounding area. Soaked black foam with wound cleanser for about 5 minutes so it does not stick to wound bed. Removed black foam gently without any difficulty. Cleansed wound bed with wound cleanser, gently dried, cleaned periwound as well. Cut black foam to fit wound bed. Covered with vac drape securely. Cut a small hole on the drape over a central area of the largest area of the black foam. Attached the trac pad onto the hole. Connected trac pad tubing to canister tubing. Turned on wound vac machine. Notified patient that machine is about to be started and to prepare to anticipate the vacuum pressure. Started machine at the pre-set 125mm/Hg setting. No air leak is observed. Patient tolerated procedure well.
